A 55-year-old woman presented with the chief complaint of pelvic pain, mostly felt on the right side. Radiography and CT scan showed a lytic lesion in the right ischium. A CT-guided biopsy was performed, and the sample revealed a spindle cell area without any malignant components. The patient underwent curettage and bone graft surgery, and the pathology report indicated spindle cell hemangioendothelioma. Spindle cell hemangioendothelioma is a rare vascular tumor, and its occurrence in bone tissue is extremely rare. We have described a unique case of intraosseous spindle cell hemangioendothelioma of the ischium.
